Printed 'Certificate of Disability for Discharge' form, 27 cm, filled in. 'Geo. C. Hardesty, of Captain Ketchum Battery ... Confederate States Artillery was enlisted ... at Mobile, Ala. on the 20th day of Feb. 1862 to serve two years. He was born in Danville in the State of Kentucky, is fifty-two years of age, five feet, four inches high, fair complexion, blue eyes, grey hair, and by occupation when enlisted a Saddler. During the last two months said soldier has been unfit for duty sixty days. ... [F]ind him incapable of performing the duties of a soldier because of age & chronic heppatitis which render him unfit for duty in any Dept. of the Government.' Accompanying and adhered to the certificate is an Affidavit of age issued by a justice of the peace in Hamilton County, Tennessee.
