Printed form, 34 cm, filled in, marked "Copy" above "Certificate of Death": "I hereby certify that Jonathan S. Blaney who was an ordinary seaman in the United States Navy while attached to the U.S.S. 'Saratoga' at Duboy Sound, Ga., ... departed this life on board the U.S.S. Saratoga at Duboy Sound, Ga., on the thirty first day of January in the year 1865 and that he died of febris typhoides as set forth in the record of his case, as follows: Jonathan S. Blaney, O.S. aged 24 years, born in Marblehead, Massachusetts. Admitted Jany 9th ... the disease had its origin in exposure while on duty in the Naval Brigade on the expedition to Pocotaligo Bridge and therefore in the line of duty" signed "Winthrop Butler, A.A. Surgeon," Further states that Blaney was five feet, five inches, dark complexion, blue eyes, dark hair and entered the U.S. Naval Service at Philadelphia on the sixteenth day of May 1865. Signed by L.P. Yorke, Paymaster and approved "E Brodhead, A. Lt Comdr Commanding U.S.S. Saratoga." Docketed on verso "Copy of certificate of death, Jonathan L. Blaney."
