- Printed form, 26 x 40 cm, filled in, "Navy Department. Sir: You are hereby appointed an acting Assistant Surgeon in the Navy of the United States, on temporary service. After having executed the enclosed oath, and returned it to the Department, with your letter of acceptance, you will report to the Commandant of the Navy Yard at Boston, Mass. for duty on board the U.S. Ship Wm. G. Anderson. I am, respectfully, your obedient servant [signed] Gideon Welles Secretary of the Navy." Addressed to "Acting Assistant Surgeon, Winthrop Butler, U.S. Navy, Boston, Mass." Handwritten notation in top left corner. Docketed on reverse.
